Si manifesta con l&#8217;invasione dei linfociti nel sangue periferico e midollare, ma ha una bassa aggressivit\u00e0. \u00c8 la forma pi\u00f9 comune di leucemia nei Paesi occidentali e colpisce principalmente gli adulti (media di 65 anni), con una leggera prevalenza negli uomini. Le manifestazioni cliniche della malattia sono causate dall&#8217;infiltrazione progressiva del midollo osseo, dei linfonodi e di altri tessuti da parte dei linfociti, oltre a alterazioni immunologiche. Macchie (ombre) di G\u00fcmprecht (MG) insieme a linfociti di piccole dimensioni ed aspetto maturo (Lm). <br>Fonte: <a href=\"https:\/\/www.amazon.it\/gp\/search?ie=UTF8&amp;tag=bmscience.net-21&amp;linkCode=ur2&amp;linkId=5cebff71c29fde6c2db15b924486a30e&amp;camp=3414&amp;creative=21718&amp;index=books&amp;keywords=Manuale%20ematologia%20SSM\" target=\"_blank\" rel=\"noopener\">Manuale di ematologia SSM<\/a>.<\/figcaption><\/figure>\n<\/div>\n\n\n<ul class=\"wp-block-list\">\n<li><span style=\"text-decoration: underline;\">Emocromo e striscio di sangue periferico<\/span> mostrano <strong>leucocitosi<\/strong>, <strong>linfocitosi<\/strong> (linfociti di piccole dimensioni e aspetto maturo con <strong>ombre di G\u00fcmprecht<\/strong>), <strong>anemia<\/strong> e <strong>trombocitopenia<\/strong>.<\/li>\n\n\n\n<li><span style=\"text-decoration: underline;\">L&#8217;esame del midollo osseo<\/span>\u00a0non \u00e8 richiesto per la diagnosi, ma pu\u00f2 mostrare\u00a0<strong>infiltrazione<\/strong>\u00a0di diverso grado da parte di piccoli linfociti, con quattro possibili pattern (nodulare, diffuso, interstiziale e misto nodulo-interstiziale).<\/li>\n\n\n\n<li>La\u00a0<span style=\"text-decoration: underline;\">citogenetica\u00a0<\/span>rivela alterazioni nell&#8217;80% dei casi, come del (<strong>13q<\/strong>),\u00a0<strong>trisomia 12<\/strong>\u00a0o, meno frequentemente, del (<strong>17p<\/strong>) che conferisce resistenza alla chemioterapia.<\/li>\n\n\n\n<li>L&#8217;<span style=\"text-decoration: underline;\">immunofenotipo<\/span>\u00a0evidenzia\u00a0<strong>linfociti B<\/strong>\u00a0(<strong>CD19+<\/strong>,\u00a0<strong>CD22+<\/strong>) con coespressione di\u00a0<strong>CD5<\/strong>,\u00a0<strong>CD23<\/strong>\u00a0e\u00a0<strong>CD20<\/strong>\u00a0(debole).<\/li>\n<\/ul>\n\n\n\n<p>Altri marcatori includono <strong>ipogammaglobulinemia<\/strong>, <strong>LDH elevata<\/strong>, <strong>test di Coombs diretto positivo<\/strong> e <strong>livelli aumentati di \u03b22-microglobulina<\/strong>. Nei pazienti giovani, \u00e8 consigliabile effettuare uno screening per un potenziale donatore allogenico sin dalla diagnosi.<\/li>\n\n\n\n<li><strong>Clorambucil<\/strong> (alchilante) con o senza <strong>rituximab<\/strong> o <strong>obinutuzumab<\/strong> per pazienti non idonei con mutazioni p53 o del (17p).<\/li>\n\n\n\n<li><span style=\"text-decoration: underline;\">Seconda linea di trattamento<\/span> con <strong>venetoclax<\/strong> (anti-bcl2) e <strong>rituximab<\/strong>, <strong>idelalisib<\/strong> (anti-PI3K-delta) o <strong>ibrutinib<\/strong> (se non usato precedentemente).<\/li>\n\n\n\n<li>Ulteriori linee di trattamento possono includere l&#8217;uso di <strong>CAR-T anti-CD19<\/strong>.<\/li>\n<\/ul>\n\n\n\n<h3 class=\"wp-block-heading\"><span class=\"ez-toc-section\" id=\"Evoluzione\"><\/span>Evoluzione<span class=\"ez-toc-section-end\"><\/span><\/h3>\n\n\n\n<p>La LLC pu\u00f2 evolvere in una <strong>leucemia prolinfocitica<\/strong> con pi\u00f9 del 55% di prolinfociti nel sangue periferico, che ha una prognosi infausta, o nella <strong>sindrome di Richter<\/strong> con trasformazione in un linfoma a grandi cellule ad alto grado. La maggior parte dei pazienti che sviluppano la malattia in giovane et\u00e0 muore a causa della neoplasia stessa e dell&#8217;immunodeficienza umorale.
