Questa evoluzione \u00e8 stata guidata da cambiamenti significativi sia nel campo della radiochimica, sia in quello tecnologico e strumentale. Tale processo di sviluppo \u00e8 ancora in corso e \u00e8 motivato principalmente dalla necessit\u00e0 di creare radiofarmaci adatti per caratterizzare una sempre pi\u00f9 ampia gamma di malattie in modo altamente specifico. \u00c8 importante notare che il concetto di &#8220;specificit\u00e0&#8221; in medicina nucleare ha un significato diverso rispetto a quello utilizzato in altre forme di diagnosi medica.<\/p>\n\n\n<div id=\"bmscience3953137827\" style=\"margin-top: 15px;margin-left: 15px;float: right;\"><a href=\"https:\/\/amzn.to\/4m9OkbU\" target=\"_blank\" aria-label=\"Progetto senza titolo\"><img loading=\"lazy\" decoding=\"async\" src=\"https:\/\/www.bmscience.net\/blog\/wp-content\/uploads\/2025\/08\/Progetto-senza-titolo.gif\" alt=\"\"  width=\"300\" height=\"300\"   \/><\/a><\/div>\n\n\n<p>Mentre la maggior parte delle procedure medico-nucleari riesce efficacemente a distinguere tra la presenza e l&#8217;assenza di una malattia, la loro capacit\u00e0 di differenziare tra diverse patologie \u00e8 spesso limitata. Ad esempio, nella <strong>scintigrafia scheletrica<\/strong>, \u00e8 difficile distinguere patologie molto diverse, come neoplasie e condizioni non neoplastiche, poich\u00e9 possono presentare immagini scintigrafiche simili. Considerazioni simili si applicano anche alle <strong>indagini PET<\/strong> nell&#8217;ambito oncologico, dove \u00e8 praticamente impossibile differenziare diversi tipi di tumori basandosi esclusivamente sulla captazione del radiofarmaco metabolico.<\/p>\n\n\n\n<p>Nel frattempo, molti radiofarmaci precedentemente utilizzati, come <strong><sup>131<\/sup>I-Sieroalbumina umana<\/strong> (usato in passato come indicatore intravascolare), <strong><sup>131<\/sup>I-Macroaggregati di albumina umana<\/strong> (per la scintigrafia polmonare perfusionale), <strong><sup>131<\/sup>I-Fibrinogeno umano<\/strong> (per la scintigrafia dei trombi intravascolari), <strong><sup>198<\/sup>Au-colloidale<\/strong> e <strong><sup>111<\/sup>InCl<sub>3<\/sub><\/strong> (per la scintigrafia epato-splenica e midollare), <strong><sup>75<\/sup>Se-Metionina<\/strong> (per la scintigrafia pancreatica e paratiroidea in combinazione con un secondo radiofarmaco), <strong><sup>197<\/sup>Hg-Clormerodrina<\/strong> (per la scintigrafia renale), <strong><sup>111<\/sup>In-Bleomicina<\/strong> (radiofarmaco oncotropo generale), vari radiofarmaci marcati con<strong><sup> 113m<\/sup>In<\/strong> (con diverse applicazioni), sono diventati obsoleti e non sono pi\u00f9 disponibili commercialmente.<\/p>\n\n\n<div id=\"bmscience687763836\" style=\"margin-top: 15px;margin-bottom: 15px;margin-left: auto;margin-right: auto;text-align: center;\"><div data-id='24174' class='amazon-auto-links aal-js-loading'><p class='now-loading-placeholder'>Caricamento&#8230;.<\/p><\/div><\/div>\n\n\n<p>Un altro importante impulso allo sviluppo di nuovi radiofarmaci diagnostici \u00e8 stata la crescente preoccupazione per la <strong>riduzione della dose<\/strong> di radiazioni ionizzanti a cui sono esposti i pazienti durante le indagini diagnostiche. A titolo esemplificativo, nella tabella in basso sono riportati i principali <strong>valori radiodosimetrici<\/strong>, espressi sia come dose effettiva al corpo intero che come dose assorbita per unit\u00e0 di radioattivit\u00e0 somministrata, relativi ai tre organi\/tessuti pi\u00f9 esposti durante l&#8217;utilizzo dei radiofarmaci diagnostici convenzionali pi\u00f9 comunemente impiegati
