La crescita del tumore prostatico \u00e8 generalmente lenta, e molti casi non presentano sintomi nelle fasi iniziali. Il rischio di sviluppare il tumore aumenta con l&#8217;et\u00e0, con una prevalenza maggiore nei soggetti con storia familiare di cancro alla prostata e in quelli di etnia africana. Altri fattori di rischio includono livelli elevati di testosterone, dieta ricca di grassi animali, e alcune mutazioni genetiche.<\/p>\n\n\n<div id=\"bmscience4177307638\" style=\"margin-top: 15px;margin-right: 15px;float: left;\"><div style=\"\r\n  width: 300px;\r\n  margin: 0 auto;\r\n  text-align: center;\r\n\">\r\n<div data-id='24157' class='amazon-auto-links aal-js-loading'><p class='now-loading-placeholder'>Now loading&#8230;<\/p><\/div><\/div><\/div>\n\n\n<p>Il segno pi\u00f9 comune in fase avanzata \u00e8 l&#8217;<strong>aumento del PSA<\/strong> (<strong>Antigene Prostatico Specifico<\/strong>) nel sangue, che pu\u00f2 essere usato come indicatore di malattia, anche se non \u00e8 specifico. La diagnosi definitiva viene effettuata attraverso biopsia prostatiche, in cui un campione di tessuto della prostata viene esaminato per rilevare la presenza di cellule tumorali.<\/p>\n\n\n\n<p>Il PSMA (<em><strong>Prostate-Specific Membrane Antigen<\/strong><\/em>) \u00e8 una proteina di membrana presente in quantit\u00e0 elevate nelle cellule tumorali prostatiche, rendendola un obiettivo ideale per l&#8217;imaging PET.<\/p>\n\n\n\n<p>Il trattamento del tumore prostatico dipende dal grado di aggressivit\u00e0 e dallo stadio della malattia, e pu\u00f2 includere chirurgia, radioterapia, ormonoterapia, o una combinazione di questi. La prognosi \u00e8 generalmente favorevole, con molte forme di tumore prostatico che progrediscono lentamente, ma ci\u00f2 dipende anche dal grado di rischio della malattia.<\/p>\n\n\n<div id=\"rtoc-2\"  class=\"wp-block-heading\">La scala di Gleason<\/h2>\n\n\n\n<p>La scala di Gleason \u00e8 un sistema di classificazione utilizzato per descrivere l&#8217;<strong>aggressivit\u00e0<\/strong> del carcinoma prostatico. Si basa sull&#8217;aspetto delle cellule tumorali osservate al microscopio e sull&#8217;analisi della loro somiglianza con il tessuto prostatico normale. 0,2 ng\/ml misurato tra 6 e 13 settimane dopo l&#8217;intervento;<\/li>\n\n\n\n<li><strong>ristadiazione per ripresa biochimica (BRC) <\/strong>dopo radioterapia ad intento radicale se PSA &gt; 2 ng\/ml al di sopra del PSA nadir post-radioterapia, con o senza terapia di deprivazione androgenica (ADT);<\/li>\n\n\n\n<li><strong>ristadiazione in paziente con persistenza biochimica (BCP)<\/strong>, definita come un PSA persistentemente elevato \u2265 0,1 ng\/mL oltre 6 settimane dopo prostatectomia;<\/li>\n\n\n\n<li><strong>ristadiazione in paziente con carcinoma prostatico resistente alla castrazione non metastatico<\/strong> (<strong>nmCRPC<\/strong>) nonostante una terapia di deprivazione androgenica (ADT) efficace. Condizioni necessarie sono l&#8217;aumento progressivo del PSA (tre incrementi consecutivi, con due aumenti del 50% rispetto al nadir), un PSA &gt; 2 ng\/mL e assenza di metastasi all&#8217;imaging convenzionale (TC o scintigrafia ossea);<\/li>\n\n\n\n<li><strong>stadiazione pre-terapia con RLT PSMA per il carcinoma prostatico metastatico<\/strong> (confermare l&#8217;idoneit\u00e0 alla RLT attraverso la documentazione dell&#8217;espressione di PSMA nelle sedi metastatiche e valutare la probabilit\u00e0 di risposta alla terapia radioligante).<\/li>\n<\/ul>\n\n\n<div id=\"bmscience1825757017\" style=\"margin-top: 15px;margin-left: 15px;float: right;\"><a href=\"https:\/\/amzn.to\/4k5ejQz\" target=\"_blank\" aria-label=\"Screenshot 2025-05-21 204822\"><img loading=\"lazy\" decoding=\"async\" src=\"https:\/\/www.bmscience.net\/blog\/wp-content\/uploads\/2025\/05\/Screenshot-2025-05-21-204822.png\" alt=\"\"  srcset=\"https:\/\/www.bmscience.net\/blog\/wp-content\/uploads\/2025\/05\/Screenshot-2025-05-21-204822.png 382w, https:\/\/www.bmscience.net\/blog\/wp-content\/uploads\/2025\/05\/Screenshot-2025-05-21-204822-300x268.png 300w\" sizes=\"auto, (max-width: 382px) 100vw, 382px\" width=\"300\" height=\"268\"   \/><\/a><\/div>\n\n\n<p>Oltre a queste indicazioni la PET con PSMA \u00e8 utile come:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>guida per la biopsia prostatica<\/strong> (migliora la localizzazione del tumore e guida le biopsie ripetute nei pazienti con sospetto elevato di carcinoma prostatico e biopsie precedenti negative);<\/li>\n\n\n\n<li>imaging del<strong> carcinoma prostatico metastatico<\/strong> (migliore accuratezza diagnostica rispetto alla scintigrafia ossea e all&#8217;imaging convenzionale);<\/li>\n\n\n\n<li><strong>monitoraggio del trattamento sistemico<\/strong> nel carcinoma prostatico metastatico.<\/li>\n<\/ul>\n\n\n\n<p>L&#8217;unica controindicazione relativa \u00e8 la <strong>claustrofobia <\/strong>o ridotta compliance del paziente a mantenere l\u2019immobilit\u00e0.<br>Controindicazione assoluta \u00e8 uno stato di <strong>gravidanza <\/strong>nei casi in cui l\u2019indagine sia richiesta nelle patologie oncologiche non prostata correlate.<\/p>\n\n\n\n<p>Evidenze scientifiche fanno emergere un incremento della espressione recettoriale nel carcinoma prostatico in corso di terapia di <strong>deprivazione androgenica<\/strong> nei pazienti ormono-resistenti (mentre non si evidenzia negli ormono-sensibili)<br>E\u2019 un fenomeno transitorio che fa riferimento alle prime 3-4 settimane dall\u2019avvio dell\u2019ADT.<br>Allo stato attuale la concomitante somministrazione di terapie sistemiche non rappresenta una controindicazione assoluta alla esecuzione della PSMA-PET in paziente con recidiva biochimica n\u00e9 si ritiene necessaria la sospensione della terapia. Bisogna tuttavia tener conto del timing dall\u2019inizio del trattamento ADT per prevenire erronee interpretazioni tipo progressione di malattia se la PET viene eseguita a breve distanza dall\u2019inizio della cura.<\/p>\n\n\n<div id=\"bmscience3205548199\" style=\"margin-left: auto;margin-right: auto;text-align: center;\"><script async src=\"\/\/pagead2.googlesyndication.com\/pagead\/js\/adsbygoogle.js?client=ca-pub-3495866718878812\" crossorigin=\"anonymous\"><\/script><ins class=\"adsbygoogle\" style=\"display:block;\" data-ad-client=\"ca-pub-3495866718878812\" \ndata-ad-slot=\"4682122636\" \ndata-ad-format=\"auto\" data-full-width-responsive=\"true\"><\/ins>\n<script> \n(adsbygoogle = window.adsbygoogle || []).push({}); \n<\/script>\n<\/div>\n\n\n<h2 id=\"rtoc-5\"  class=\"wp-block-heading\">Interpretazione delle immagini (F- vs Ga-PSMA)<\/h2>\n\n\n\n<p>Per quanto riguarda i <strong>vantaggi dei PSMA fluorati<\/strong> vi sono: elevata emivita, elevata disponibilit\u00e0 di dosi se si possiede un ciclotrone, scarsa escrezione con le urine (consente miglior accuratezza diagnostica sulla recidiva locale, miglior identificazione dell&#8217;infiltrazione retto-vescicale e delle vescichette seminali).<\/p>\n\n\n\n<p>Le tempistiche di acquisizione:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>PSMA legati al <sup>68<\/sup>Ga<\/strong>: dopo 50-60 minuti dalla iniezione;<\/li>\n\n\n\n<li><strong>PSMA <\/strong><strong>florurati<\/strong>: in genere vengono consigliati <strong>120 minuti <\/strong>dopo l\u2019iniezione ma la lunga tempistica di attesa diventa mal tollerata dal paziente. La maggior parte dei Centri acquisiscono dopo <strong>90 minuti <\/strong>per ragioni pratiche, tuttavia alcuni recenti lavori suggeriscono una acquisizione precoce a <strong>60 minuti <\/strong>e poi eseguire una seconda rilevazione tardiva dopo 30 minuti nelle sedi di interesse per incrementare la specificit\u00e0 dell\u2019esame (un incremento del SUVmax &gt; 10% rispetto a stazionariet\u00e0 o riduzione \u00e8 maggiormente predittivo di lesione neoplastica).&nbsp;<\/li>\n<\/ul>\n\n\n<div class=\"wp-block-image\">\n<figure class=\"alignright size-full is-resized\"><img loading=\"lazy\" decoding=\"async\" width=\"310\" height=\"286\" src=\"https:\/\/www.bmscience.net\/blog\/wp-content\/uploads\/2024\/12\/Ga-PSMA-vs-F-PSMA.jpg\" alt=\"\" class=\"wp-image-21569\" style=\"width:337px;height:auto\" srcset=\"https:\/\/www.bmscience.net\/blog\/wp-content\/uploads\/2024\/12\/Ga-PSMA-vs-F-PSMA.jpg 310w, https:\/\/www.bmscience.net\/blog\/wp-content\/uploads\/2024\/12\/Ga-PSMA-vs-F-PSMA-300x277.jpg 300w\" sizes=\"auto, (max-width: 310px) 100vw, 310px\" \/><figcaption class=\"wp-element-caption\">Ga-PSMA vs F-PSMA<\/figcaption><\/figure>\n<\/div>\n\n\n<p>Entrambi i traccianti presentano siti di <strong>fisiologica captazione:<\/strong><\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><span style=\"text-decoration: underline;\">intensa<\/span>: in corrispondenza di ghiandole salivari maggiori e minori, ghiandole lacrimali, milza, interstizio renale da riassorbimento al tubulo contorto distale, colecisti, vie biliari, intestino tenue;<\/li>\n\n\n\n<li><span style=\"text-decoration: underline;\">moderata<\/span>: a livello del fegato, pancreas, gangli e paragangli del sistema ortosimpatico.<\/li>\n<\/ul>\n\n\n\n<p>Ulteriori siti di accumulo del radiofarmaco, al di fuori della fisiologica distribuzione corporea, sono:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><span style=\"text-decoration: underline;\">reperti neoplastici;<\/span><\/li>\n\n\n\n<li><span style=\"text-decoration: underline;\">reperti non neoplastici:<\/span> flogosi, ad esempio captazione a livello di trachea e bronchi o captazioni del tutto aspecifiche e non correlate a processi infiammatori.<\/li>\n<\/ul>\n\n\n<div class=\"wp-block-image\">\n<figure class=\"alignleft is-resized\"><img loading=\"lazy\" decoding=\"async\" width=\"412\" height=\"617\" src=\"https:\/\/www.bmscience.net\/blog\/wp-content\/uploads\/2024\/12\/PSMA.jpg\" alt=\"\" class=\"wp-image-21572\" style=\"width:259px;height:auto\" srcset=\"https:\/\/www.bmscience.net\/blog\/wp-content\/uploads\/2024\/12\/PSMA.jpg 412w, https:\/\/www.bmscience.net\/blog\/wp-content\/uploads\/2024\/12\/PSMA-200x300.jpg 200w\" sizes=\"auto, (max-width: 412px) 100vw, 412px\" \/><\/figure>\n<\/div>\n\n\n<p>La spiegazione di questa captazione \u00e8 determinata dal fatto che la molecola di PSMA non \u00e8 espressa solo dalla prostata ma \u00e8 presente anche in condizioni di benignit\u00e0 riconducibili all\u2019attivit\u00e0 osteoblastica (osteoartrite, displasia fibrosa, riparazione delle fratture, dopo radioterapia e nella malattia di Paget).<\/p>\n\n\n\n<p>L\u2019accumulo non specifico \u00e8 invece correlato all&#8217;elevata espressione di recettori nelle <strong>cellule endoteliali <\/strong>della <strong>neovascolarizzazione <\/strong>dell&#8217;<strong>angiogenesi <\/strong>e alla elevata permeabilit\u00e0 delle cellule infiammatorie nei siti di <strong>flogosi <\/strong>(legame con il recettore beta del folato presente nei macrofagi) che perci\u00f2 rappresentano falsi positivi.<\/p>\n\n\n\n<p>Per tali motivi la captazione del PSMA \u00e8 molto specifica ma non esclusiva del tumore della prostata.<\/p>\n\n\n\n<figure class=\"wp-block-table\"><table class=\"has-fixed-layout\"><thead><tr><th>Altre neoplasie benigne\/maligne<\/th><th>Situazioni non neoplastiche<\/th><\/tr><\/thead><tbody><tr><td>Carcinoma a \u00abcellule chiare\u00bb del rene (rara)<br>Carcinoma uroteliale (rara)<br>HCC (frequente)<br>Gliomi e meningiomi (rara)<br>Neoplasie polmonari (rara)<br>Tumore desmoide o fibromatosi aggressiva (comune)&nbsp;<br>Morbo di Paget (rara)<br>Angiolipoma (comune)<br>LNH follicolare (rara)<br>Mieloma multiplo (comune)<br>Carcinoma mammario (rara)<br>Schwannoma (rara)<br>Timoma (rara)<\/td><td>Splenosi (comune)<br>Gangli e paragangli stellati, celiaci, ipogastrici e presacrali (frequente)<br>Sarcoidosi (comune)<br>Siti di flogosi reattiva linfonodale (frequente)<br>Displasia fibrosa ossea (frequente)<br>Artrite (frequente)<br>Neurofibromatosi (rara)<br>Esiti di fratture (frequente)<br>Esiti di radioterapia (comune)<br>Angioma osseo (comune)<br>Poliangite eosinofila con granulomatosi (rara)<br>Infezioni polmonari (comune)<br>Ginecomastia (comune)<br>Fascite nodulare (rara)<\/td><\/tr><\/tbody><\/table><\/figure>\n\n\n\n<p>Ulteriori captazioni aspecifiche, anche se focali, si possono avere a livello <strong>scheletrico<\/strong>, specie con F-PSMA. La causa non \u00e8 stata ancora identificata, ma alcuni studi hanno dimostrato che il valore del SUVmax pu\u00f2 contribuire ad orientare verso una maggiore o minore probabilit\u00e0 di lesione neoplastica rispetto a sito di captazione aspecifica. E&#8217; stato stabilito un cut-off di SUVmax pari 7. Tuttavia vi \u00e8 l&#8217;eccezione del <strong>tumore desmoide<\/strong> che pu\u00f2 avere SUVmax &gt; 10.<\/p>\n\n\n\n<p>La probabilit\u00e0 di descrivere <strong>veri positivi <\/strong>\u00e8 funzione dei valori della biochimica:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>38% per valori di PSA &lt; 0,5 ng\/ml;<\/li>\n\n\n\n<li>57% per valori di PSA tra 0,5 e 1 ng\/ml;<\/li>\n\n\n\n<li>84% per valori di PSA tra 1 e 2 ng\/ml;<\/li>\n\n\n\n<li>86% per valori di PSA tra 2 e 5 ng\/ml;<\/li>\n\n\n\n<li>97% per valori di PSA &gt; 5 ng\/ml.<\/li>\n<\/ul>\n\n\n\n<p>Non tutti i tumori della prostata esprimono elevata densit\u00e0 di recettori per il PSMA. Fino al 5% degli ADK della prostata non \u00e8 rilevabile con la metodica, in particolare le<strong> forme aggressive <\/strong>o con <strong>sdifferenziazione neuroendocrina<\/strong>. In corso di terapie sistemiche i pazienti metastatici resistenti alla castrazione potrebbero presentare&nbsp;lesioni con ridotta espressione&nbsp;recettoriale.
