L\u2019incidenza \u00e8 maggiore nelle donne&nbsp;<strong>oltre i 65 anni<\/strong>. Purtroppo,&nbsp;<strong>la met\u00e0 dei casi viene identificata in fase avanzata<\/strong>&nbsp;(stadio III o IV). La&nbsp;<strong>sopravvivenza migliore<\/strong>&nbsp;si osserva nelle pazienti in cui \u00e8 possibile effettuare una&nbsp;<strong>diagnosi precoce<\/strong>&nbsp;o una&nbsp;<strong>chirurgia di citoriduzione ottimale<\/strong>.<\/p>\n\n\n\n<p>Tuttavia, la diagnosi precoce \u00e8 rara e spesso avviene&nbsp;<strong>casualmente<\/strong>, ad esempio durante un\u2019<strong>ecografia addominale o pelvica<\/strong>&nbsp;eseguita per altri motivi. In assenza di test di screening affidabili, il sospetto clinico emerge&nbsp;<strong>solo in fase avanzata<\/strong>, poich\u00e9&nbsp;<strong>i primi sintomi clinici sono aspecifici e non allarmanti<\/strong>.<\/p>\n\n\n\n<p>La&nbsp;<strong>PET con fluorodesossiglucosio (FDG)<\/strong>&nbsp;pu\u00f2 essere&nbsp;<strong>raramente utile<\/strong>&nbsp;nella diagnosi differenziale del carcinoma ovarico primario, a causa della&nbsp;<strong>possibile sovrapposizione dell\u2019assorbimento di FDG<\/strong>&nbsp;tra condizioni&nbsp;<strong>benigne, fisiologiche e maligne<\/strong>.<\/p>\n\n\n\n<p>Nelle donne in et\u00e0 fertile, l\u2019interpretazione dell\u2019assorbimento di FDG deve essere&nbsp;<strong>attenta e contestualizzata<\/strong>&nbsp;rispetto all\u2019et\u00e0 e allo stato mestruale, per evitare falsi positivi. Al contrario,&nbsp;<strong>nelle donne in postmenopausa<\/strong>, qualsiasi captazione anomala a livello degli annessi deve essere considerata&nbsp;<strong>sospetta per neoplasia<\/strong>, e richiede ulteriori indagini come&nbsp;<strong>ecografia transvaginale (TVUS)<\/strong>&nbsp;o&nbsp;<strong>risonanza magnetica (MRI)<\/strong>.<\/p>\n\n\n<div id=\"bmscience590242725\" style=\"margin-top: 15px;margin-left: 15px;float: right;\"><a href=\"https:\/\/amzn.to\/3HvxfJO\" target=\"_blank\" aria-label=\"Version 1.0.0\"><img loading=\"lazy\" decoding=\"async\" src=\"https:\/\/www.bmscience.net\/blog\/wp-content\/uploads\/2025\/05\/e9df8a88-7895-4804-8211-20398110afe0.jpeg\" alt=\"\"  srcset=\"https:\/\/www.bmscience.net\/blog\/wp-content\/uploads\/2025\/05\/e9df8a88-7895-4804-8211-20398110afe0.jpeg 640w, https:\/\/www.bmscience.net\/blog\/wp-content\/uploads\/2025\/05\/e9df8a88-7895-4804-8211-20398110afe0-80x300.jpeg 80w, https:\/\/www.bmscience.net\/blog\/wp-content\/uploads\/2025\/05\/e9df8a88-7895-4804-8211-20398110afe0-273x1024.jpeg 273w, https:\/\/www.bmscience.net\/blog\/wp-content\/uploads\/2025\/05\/e9df8a88-7895-4804-8211-20398110afe0-546x2048.jpeg 546w\" sizes=\"auto, (max-width: 640px) 100vw, 640px\" width=\"180\" height=\"675\"   \/><\/a><\/div>\n\n\n<p>La&nbsp;<strong>stadiazione del carcinoma ovarico<\/strong>&nbsp;si basa sull\u2019<strong>esame istopatologico<\/strong>&nbsp;della lesione primaria e deve includere la valutazione di:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>utero e ovaio controlaterale;<\/li>\n\n\n\n<li>linfonodi pelvici e retroperitoneali;<\/li>\n\n\n\n<li>omento e peritoneo.<\/li>\n<\/ul>\n\n\n\n<p>Le&nbsp;<strong>metastasi a distanza<\/strong>&nbsp;sono meno frequenti alla diagnosi iniziale. La&nbsp;<strong>sopravvivenza<\/strong>&nbsp;\u00e8 strettamente correlata alla&nbsp;<strong>rimozione chirurgica completa di tutte le lesioni visibili<\/strong>&nbsp;<\/p>\n\n\n\n<p>La&nbsp;<strong>PET-FDG \u00e8 indicata nella stadiazione<\/strong>, poich\u00e9 contribuisce significativamente all\u2019identificazione di&nbsp;<strong>lesioni addominali e pelviche<\/strong>&nbsp;e alla valutazione dell\u2019<strong>interessamento peritoneale<\/strong>. Questo supporto pu\u00f2&nbsp;<strong>guidare la chirurgia<\/strong>&nbsp;e migliorare i risultati della citoriduzione.<\/p>\n\n\n\n<p>Rispetto alla&nbsp;<strong>TC con mezzo di contrasto (CECT)<\/strong>&nbsp;o alla&nbsp;<strong>MRI<\/strong>, la PET-FDG pu\u00f2 offrire un&nbsp;<strong>valore aggiunto<\/strong>&nbsp;nella rilevazione di&nbsp;<strong>metastasi extra-addominali<\/strong>, che sono rare ma associate a&nbsp;<strong>prognosi peggiore<\/strong>.<\/p>\n\n\n\n<p>Il follow-up si basa su:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>dosaggio plasmatico del CA 125<\/strong>;<\/li>\n\n\n\n<li><strong>esame clinico e obiettivo<\/strong>.<\/li>\n<\/ul>\n\n\n\n<p>In caso di sospetto di recidiva (aumento del CA 125 o comparsa di sintomi), la&nbsp;<strong>CECT o la MRI addominale<\/strong>&nbsp;sono generalmente le prime indagini consigliate.<\/p>\n\n\n\n<p>Il ruolo della&nbsp;<strong>PET-FDG nella ristadiazione<\/strong>&nbsp;\u00e8 in crescita: studi dimostrano una&nbsp;<strong>migliore accuratezza rispetto alla CECT<\/strong>, soprattutto nei pazienti con&nbsp;<strong>quadri atipici<\/strong>, come:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>aumento isolato del CA 125;<\/li>\n\n\n\n<li>sintomi clinici suggestivi di recidiva con CA 125 normale.<\/li>\n<\/ul>\n\n\n\n<p>La PET-FDG \u00e8 particolarmente utile quando&nbsp;<strong>CECT e MRI risultano negative<\/strong>, ma vi \u00e8 un&nbsp;<strong>forte sospetto clinico di recidiva<\/strong>, grazie alla sua&nbsp;<strong>elevata sensibilit\u00e0 nel rilevare metastasi a distanza<\/strong>.<\/p>\n\n\n\n<div class=\"wp-block-advgb-adv-tabs advgb-tabs-wrapper da notare il coinvolgimento di un linfonodo sovraclaveare sinistro.<\/p>\n\n\n\n<pre class=\"wp-block-verse\">Il carcinoma ovarico si diffonde solitamente ai linfonodi locali e alla cavit\u00e0 peritoneale; la diffusione sovradiaframmatica \u00e8 considerata metastasi a distanza. captazione di FDG a livello del peritoneo, riferibile a infiltrazione neoplastica peritoneale.<\/p>\n\n\n\n<pre class=\"wp-block-verse\">La TC con mezzo di contrasto \u00e8 ampiamente utilizzata per il rilevamento della carcinomatosi peritoneale. Tuttavia, la sua sensibilit\u00e0 dipende fortemente da dimensioni, localizzazione e morfologia del tumore. La carcinomatosi peritoneale, infatti, spesso non viene rilevata nelle sue fasi iniziali tramite TC. Sebbene la sensibilit\u00e0 nella valutazione linfonodale sia intorno al&nbsp;<strong>60%<\/strong>&nbsp;(non sufficiente per sostituire la linfadenectomia), la PET-FDG \u00e8 utile per:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>individuare metastasi a distanza<\/strong>;<\/li>\n\n\n\n<li><strong>modificare l\u2019approccio terapeutico<\/strong>, passando da un trattamento radicale a uno palliativo, se necessario.<\/li>\n<\/ul>\n\n\n\n<p>Le&nbsp;<strong>recidive<\/strong>&nbsp;si verificano principalmente&nbsp;<strong>entro i primi 3 anni<\/strong>&nbsp;dalla diagnosi. In contesti clinici appropriati, l\u2019uso della PET pu\u00f2&nbsp;<strong>influenzare la strategia terapeutica fino al 30% dei casi<\/strong>, migliorando la precisione del
